The NH Mom Hub is eager to continue working with the regional coalitions of the NHPQC to ensure we capture all of the groups, providers, agencies, and services that support pregnant and parenting people. NH Mom Hub has collected a list of perinatal resources available across the state into one comprehensive and filter ready list.

Please refer to NH Mom Hub to provide additional navigation and peer support!  

NH Mom Hub is a statewide, central access point to guide patients and clinicians to resources and referrals for maternal mental health. 

  • Care navigation is layered with peer support.
  • We listen with a critical ear to identify underlying social determinants of health that may be impacting the mother’s well-being, allowing us to tailor resources to their needs. 
  • We can serve as a bridge with peer support while they are awaiting an appointment with a trained maternal mental health therapist or prescriber.
  • If their needs escalate, we can support navigation to a higher level of care.
